Please complete the Application for Service and the Cross Connection Survey forms linked below and submit them both to the East Pierce office at least one business day prior to your desired start date.

Please be sure to include the following information on your Application for Service:
- Address where service is to start
- Start date
- Your date of birth
- Last four digits of your Social Security number
- Day and evening telephone numbers
- Email address
- Billing address, if different than service address
- Signatures of all applicants (must be on both forms)
Start Service Forms
Stop Water Service
To stop your water service, please call or email our Customer Service Center at least one business day prior to the day you would like to stop service (Monday through Friday only, excluding holidays). Please stay on the call with our Customer Service Representative to receive a confirmation number. Please keep this confirmation number until you receive your final bill. If you email us, we will respond to acknowledge receipt of your request. If you do not receive an email from us within two business days of your request, please contact us to confirm receipt.

Transfer Water Service
Please call our Customer Service Center at (888) 490-3741 at least one business day prior to the date you would like to stop service at your existing address, and please provide a new Application for Service and Water Use Questionnaire for the new service address at least one business day prior to your start date.
